Job description FARSI SPEAKING PERFERRED The Registered Nurse must have ADHC/CBAS experience preferably in Orange County. Farsi speaking preferred RESPONSIBILITIES AND DUTIES: 1. Assess the nursing needs of the participant as a member of the multidisciplinary assessment team. Develop an individualized plan of care for each participant including short term and long term goals and will incorporate person-centered planning during the assessments and in the development of the care plan. 2. All assessments will expressed by participant/family members/caregiver/legal representative and their needs, goals, and desired outcome, using their own words, and 3. Document all skilled nursing care on the day of service and quarterly signed and dated progress notes. 4. Provide supportive and restorative nursing serves as defined in plan of care. 5. Develop the nursing component for continence training program. 6. Monitor, administer, and record prescribed medications according to regulations. 7. Observe and monitor health status of all participants. 8. Serve as liaison to participants personal physicians; will keep personal physicians informed of any marked change in the participant’s condition. 9. Educate staff about disease processes, physical care of the participants and infection control procedures. Is also responsible for providing health education and/or in-service training to participants. 10. Responsible for Medication management and compliance with regulatory requirements for supervising and maintaining records of prescription drugs stored at the Center. 11. Will assess participants to ensure they are capable of self-administering their own medications, in addition to monitoring and providing training to those participants in self-administration of their medications when appropriate. 12. Prior to supervising the maintenance therapy programs, the RN is required to be trained by the physical and occupational therapists in the supervision of these program. The RN will subsequently supervise the provision of the therapy maintenance program with the agreement of physical and occupation therapists. 13. Conform to all applicable policies of the Center and attend the orientation program offered by the Center. 14. Comply with all health and safety standards including licensing, fire and disaster preparedness regulations and agency policies. 15. Provide health education and counseling to the participant and the family as indicated in the plan of care. 16. Provide assistance to the participant in obtaining needed medical services from other health care providers such as the participants primary care physician. 17. Develop protocols for and supervise nursing students. 18. Completing initial assessments for every participant. 19. Completing quarterly progress notes for every participant. 20. Completing reassessments at least every six months. Please be aware that reassessments may need to be completed more often than every six months due to a change in the participant’s status, a participant’s length absence, following hospitalization. 21. Documenting any marked changes in the participant’s condition and/or any unusual observations either made by the nurse or reported by other staff in the participant’s health record. 22. Supervision of the licensed vocational nurse (LVN) and/or program aides. QUALIFICATIONS: 1. Registered nurse must have a current Nursing License. 2. Must provide Chest X-ray or test for tuberculosis infection that is recommended by the federal Centers for Disease Control and Prevention (CDC) and licensed by the federal Food and Drug Administration (FDA) performed not more than 12 months prior to employment or within 7 days of employment. 3. Must provide health examination signed by the examining physician or person lawfully authorized to perform such examination which indicates: a. Employee is physically qualified to perform duties. b. Employee is free from any condition that would create a hazard
