Licensed Vocational Nurse (LVN) Job Description
Hospice Coordinator collaboratively works closely with our interdisciplinary team (nurses, social workers, chaplains, doctors, therapists, volunteers, etc.) to ensure hospice patients receive the best possible care within the parameters of the plan of care. Occasionally, as the need arises, direct patient care may also be involved. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES ~Coordinate patient care with nurses, social workers, chaplains, and other members of the interdisciplinary team involved in patient care ~Staffing and scheduling ~QAPI duties, not limited to data and notes corrections, as assigned by DPCS. ~Coordination of QAPI and participation in QAPI meetings ~Manage performance Improvement projects ~Establish assignments of patient / family referrals and admissions, to include possible completion of intakes and assessments ~Assists Primary Care Nurses with, and monitors, patient / family education ~Evaluates all Nursing Staff activities, patient / family assignments, staffing patterns, and schedules, including the HHAs daily supervision ~Participates in staff training and education ~Works with the Director of Patient Care Services and staff clinicians to gather, collate, and report patient care summary information to Case Managers ~Regularly participates in interdisciplinary group meetings and assures attendance and record reviews that meetings are conducted in accordance with Agency and CMS regulations. ~Performs direct patient care including home assessments as required ~Performs clinical duties as needed, including patient / family visits, field supervision, and evaluation of care to facilitate teamwork within the organization. POSITION QUALIFICATIONS ~ Graduate of an accredited LPN / LVN program. ~ Three years nursing experience. Community health / hospice or medical surgical experience is preferred. ~ Currently licensed as an LPN / LVN in California. ~ Compliance with accepted professional standards and practice. ~ Demonstrates good verbal and written communication, and organization skills. ~ Driver's License with an automobile that is insured in accordance with state and/or organizations requirements and is in good working order. ~ Possesses and maintains current CPR certification. ~ Understands philosophy of hospice and palliative concept. Also understands needs of the terminally ill.
