Responsibilities •Carries out all aspects of patient care and other assignments as directed by supervising Registered Nurse •Observes whether specialized services may be needed and communicates to an RN or appropriate Medical Staff member •Reports changes in patients condition or behavior to an RN or Medical Provider •including agitation, high-risk behaviors, changes in vital signs, and eating, sleeping, and toileting patterns •Intervenes appropriately with patients experiencing a behavioral crisis situation •Initiates and/or participates in the provision of emergency medical care when indicated •Administers Med Pass to patients, ensuring that all medication is legally within their expiration dates •Documents medication variances and/or adverse drug reactions on the appropriate form and notify the patient, practitioner, and facility leadership as directed per policy •Assisting with Admissions & Discharge Requirements: LVN Active License in CA New Grads welcomed LTC exp desirable Pass Background check -COVID-19 Vaccinated
