Family run business looking for reliable LVN's to join our team on a PRN basis. Home health experience is preferred but willing to train the right candidate. • Provide skilled nursing care to patients in their homes, including wound care, medication administration, and monitoring of vital signs • Develop and implement care plans for patients in coordination with other members of the healthcare team • Educate patients and their families about their conditions and treatment plans • Monitor patients' progress and report any changes to the RN or physician • Administer medications and treatments as prescribed by a physician • Observe, document and report patient's condition and reaction to drugs, treatments, and significant incidents • Communicate with other members of the healthcare team to coordinate patient care
