Family run business looking for PRN, PT and FT nurses to help the company grow and maintain a strong communication skills with office for needs. • Conduct initial and ongoing assessments of patients' health status and needs • Develop and implement individualized care plans for patients • Administer medication and treatments as prescribed by physicians • Monitor and document patients' progress and report any changes to the physician or healthcare team • Provide patient and family education on disease management, self-care, and discharge planning • Collaborate with other healthcare professionals to coordinate care and ensure continuity of care • Participate in ongoing education and training to stay current with the latest advances in home health care
