Licensed Vocational Nurse (LVN) Job Description
Multiple positions open for per diem Continuous Home Care (CHC) LVNs to provide shift care for hospice patients in the home or Residential Care Facility (RCFE). Most patients are in the Inland Empire and Pomona Valley Region. LVN must provide direct patient care under the direction of an RN; must be proficient in caring for senior population, and possess a good understanding of hospice philosophy and approach to care. Shift may vary from 4 to 12 hour shifts per assignment.
LVN must have at least 1 year experience in the past 24 months; preferred experience in home health, hospice and nursing home. Candidate must have a good assessment skills, basic computer skills for EMR documentation, the ability to travel to the patient's home with dependable transportation, and good communication skills.
