Family Owned Home Health Agency seeking RN for per diem work

Position Summary Extra Hand is seeking a RN willing and able to provide in-home skilled nursing care to patients in the Inland Empire. The position is part-time, per diem work with the opportunity to move into full time, if interested. Qualifications • Valid RN License to practice in CA • Minimum of one (1) year nursing experience is preferred • Med Surg, ICU, ER, Acute Experience, Home Health experience is a plus • Current BLS/CPR certification • Valid CA Driver's license • Background check • Current TB test Job Responsibilities • Provide admission, evaluation and follow-up skilled nursing visits for home health patients • Provide quality hands-on care, management and teaching of patient's care plan in accordance with physician orders, under Clinical Manager's supervision • Coordinate appropriate care, involving all the other healthcare team members (such as Physical Therapists, Occupational Therapists, Home Health Aides and Medical Social Workers) • Report patient's progress to physician and Clinical Manager as required to maintain continuity of care • Implement plan of care with patient and family to assist them in achieving optimal outcomes About Extra Hand Extra Hand In-Home Care is a family and veteran owned home health agency based in Rancho Cucamonga, CA.
