FULL TIME HOSPICE AND PALLIATIVE RN CASE MANAGERS NEEDED

RN with experience in hospice highly preferred. Will require ability to case manage 15 to 18 patients caseload including admission and leading team of LVNs, CHHA, MSW and Chaplain under the direction of Director of Nursing. Good understanding of hospice philosophy of care and has the ability to develop and implement effective individualized hospice plans of care with the hospice team. Requires the ability to use a computer and navigate EHR. Good communication skills are required. Willing to train. RN completes a comprehensive assessment re-assessment of patient and family to determine hospice needs; provides direct patient care, develops and coordinates with interdisciplinary team plan of care; supervises LVNs, and Hospice Aides; serves as the RN case manager to facilitate coordination of care between hospice team and other health care setting including SNF; provides ongoing education related to hospice care. Must have at least 1 year experience under RN license in acute care settings.
