Licensed Vocational Nurse

Responsibilities • Provide Direct Care to patients in their home • Under the direction of the RN, assist in identifying the physical and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations • Participate in planning and implementing care in conjunction with the RN • Provide care utilizing infection control measures that protect both the staff and patient • Assure continuity of quality patient care delivered with appropriate documentation • Prepare regular clinical/progress notes and summaries • Maintain client confidentiality • Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury, or significant change in the patient's condition • Participate in personal and professional growth and development. • Perform other duties as assigned by a registered nurse • Use equipment and supplies effectively and efficiently.
