Licensed Vocational Nurse (LVN) Job Description
Job Title: Licensed Vocational Nurse/Licensed Psychiatric Technician The LVN/LPT staff will work with individuals with developmental disabilities in a community based, residential facility, in the medical and health functions of each client in the facility(s) assigned and will perform all necessary duties pursuant to the job responsibilities. Successful completion of LVN/LPT programs with CA issued license. Fingerprint clearance and successful completion of physical examination and TB tests are required. The LVN/LPT must have the ability to walk and stand long hours. Able to bend, squat, push and lift frequently. Able to lift – up to 50 pounds occasionally and able to assist and individual with unsteady gait. May occasionally walk on slippery or uneven surfaces. May work with individual(s) with aggressive behaviors and verbally abusive. RESPONSIBILITIES/DUTIES DAILY: 1. Licensed staff will be in the facility at least 8 hours per day, 7 days a week. 2. In the beginning of the shift, licensed staff will read communication book to ensure continuance of care and gain understanding of all clients’ condition. 3. Licensed staff to ensure all medications were administered through checking the bubble packs of all clients and review the Medication Administration Records (MAR) 4. Any discrepancy on the bubble pack and MAR are reported to Primary Care Physician, RN and QIDP. 5. Medication pass to all clients on timely manner. Read parameter before giving any medication i.e check blood pressure prior giving hypertension medication. 6. Make medical appointment as needed and document on “Medical Appointment” folder. 7. Take clients to medical appointment. 8. Follow up on and implement change of orders and recommendations from medical appointment. 9. Ensure daily body check for all clients, any findings, please follow the Staff Report Procedure 10. Monitor staff as indicated or as needed during medication pass and or mealtime. 11. Help staff as needed with ADL’s, implementation of plan(s) and interaction between staff and clients. 12. Daily charting on each client, document any acute health issue/problem daily until resolved. 13. Check medication cabinet, ensure the external and internal medication are stored separately, check for expired medication, check for medication supplies. 14. Order re-fill medications to the pharmacy when medications are low. 15. Communicate with RN for any change of client condition and/or any issue related to medical and health matters. WEEKLY: 1. Complete weekly summary 2. Check all physician orders for the need of lab, immunization or other consultation. 3. Check MAR Review if any weekly orders i.e client’s weight are followed through 4. Review and assure accuracy of MAR, weight, vital sign, etc. 2. Prepare MAR and Physician Order for upcoming months. MAR’s must be audited and sent back to Pharmacy for corrections, if needed. 3. Cycle medications must be checked and corrected. 4. Ensure that day program has a copy of current physician order and MAR if medication is being given at the day program. 5. Ensure that all Physician orders are signed my doctor every month. 6. Ensure that all applicable forms/logs are up to date such as seizure record, vital sign, weight, infections, etc. 7. Prepare and present training to the staff (as needed) 8. Attend mandatory in-services and meetings. QUARTERLY: 1. Ensure recommendations from health/medical professional are being carried through. 2.
