Personal Care Assistant (PCA) Job Description
Responsibilities • This position performs in-home assessments, in-home provider orientations and supervisory visits in Houston and surrounding areas utilizing an agency vehicle • Responsibilities also include setup and maintenance of client charts, assisting with staffing, and other miscellaneous duties within the departments • Ensures client and employee privacy and confidentiality pursuant to HIPAA Privacy Rules • Complete initial evaluation of assistance needed by client to achieve activities of daily living • Monitoring hospitalizations and / or all suspensions • Assessing and confirming service plan task meet clients’ needs • Ensuring task are not duplicated with any other services received by the client • Ensure Coordination of Care with required forms and communication • Scheduling according to service plans
