1. Comprehensive Initial Assessments
2. Bi weekly Assessments and Supervisory Visits
3. Recertification Assessments
4. PRN visits, or follow up Visits
LVNS
1. PRN Visits or Follow up visits
CHHA / CNA
1. ADL assistance including bathing, grooming
Requirements:
1. Valid license with hospice experience preferred
2. Bilingual a plus
3. Proficient with Hospicemd
