Registered Nurse (RN) Job Description
Job Description As a member of the Interdisciplinary Team the Hospice RN is an experienced Registered Nurse who provides skilled end of life care in the community setting. The Hospice RN is responsible for promoting the Hospice philosophy. This individual facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others. • Participate in a prescribed orientation and in-service process, which results in a knowledge and skill level necessary for role fulfillment. • Provide for the safety needs of patients, staff, and public by: • Notifying appropriate staff person of an unsafe area and modifies unsafe area if feasible. • Communicating and/or consulting with the Director of Nursing Services to solve environmental and/or safety problems. • Participating in the regular review of new equipment and procedures. • Provide direct hospice nursing services to patients in the home setting. • Perform needs assessment of patients and families through collection and documentation of a data base. QUALIFICATION AND REQUIREMENTS • At least 12 months of RN experience • Completion of a professional nursing program accredited by the National League for Nursing • Current RN License, CPR • Must have a valid driver’s license and a willingness to use personal vehicle in the course of employment. • Automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36-month period). • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
