Registered Nurse (RN) Job Description
JOB TITLE : Nurse/Direct Care REPORTS TO : Clinical Director, Clinical Program Director, Director of Corporate Compliance, Consulting Psychiatrist, Nursing Supervisor, and Psychiatric Mental Health Nurse Practitioner SUPERVISES : N/A EMPLOYEE STATUS: Non-exempt SUMMARY OF POSITION RESPONSIBILITIES: The Nurse is responsible for nursing assessments, monitoring and support of Creative Care, Inc. clients, assisting the Consulting Psychiatrist/Psychiatric Mental Health Nurse Practitioner with consultations, and collaborating with other nursing staff on continuation of client care. Follow facility nursing protocols and duties. ESSENTIAL DUTIES: - Conducting nursing assessments of clients, screening for mental health needs and other health-related concerns. - Follow new client intake protocols and procedures, as well as provide a complete and comprehensive orientation for new clients. - Based on preliminary assessment and/or diagnosis, recommends the next level of treatment and liaisons with appropriate staff to ensure continuity of care. - Maintain licensing and renewal responsibilities. - Implements facility protocols and monitors self – administration of medications and assists with processing laboratory requests consistent with Consulting Psychiatrist orders. - Evaluates and documents the efficacy of treatments and/or medications taken. - Functions as a member of the multidisciplinary treatment team, making appropriate referrals, advocating for patient needs, coordinating care delivery, and maintaining/improving quality standards. - Orders, receives and distributes pharmaceuticals and medical supplies as necessary. - Assists in the development of nursing policies, practices and procedures as related to the Nursing department. - Collaborates with the treatment team. - Coordinate with medical staff in assisting in scheduling appointments. - Facilitate new admission intake protocols. - Works closely with Consulting Psychiatrist/Psychiatric Mental Health Nurse Practitioner to ensure a continuity of care coverage for all clients, including telephone discussions of medication orders. - Perform other duties as assigned. - Administer Urine Analysis (UA); perform proper storage of the sample; accurately complete information on the appropriate UA form; assure proper transferring of UA sample to the appropriate testing authority. - Complete nursing notes during the assigned shift and chart on all clients in EMR and Communication Binders. - Perform receptionist duties and relay telephone messages promptly to various staff members and clients. - Complete and document rounds on all clients. - Transport clients to appointments as needed. - Send client records in accordance with HIPAA regulations and company policy and procedures. LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS: Minimum one (1) years of experience providing nursing care in a similar setting preferred. Demonstrated leadership and adaptive problem solving skills. Knowledge of confidentiality guidelines and practices. Strong oral communication skills; excellent interpersonal communication skills. Ability to work within a multidisciplinary team, in a multi-cultural/multi-lingual environment. REQUIRED CERTIFICATION/LICENSE: Valid and current RN license to practice nursing in the state of California. BLS Certification. PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle. WORKING CONDITIONS: Work is performed in an office or other environmentally controlled room; Work exposes incumbent to contagious or infectious diseases; Work may expose incumbent to dangerous and volatile situations; and Work exposes incumbent to possible bodily injury.
