The Home at Hearthstone is the premier Skilled Nursing and Long-term care facility in Cincinnati! We host residents from Cincinnati area and love what we do! POSITION QUALIFICATIONS: EDUCATION AND EXPERIENCE: Graduate of an accredited Nursing Program. Ability to add and subtract and perform basic mathematics as it relates to the position is required. Ability to read/write/comprehend the English language in written & verbal form required. Ability to communicate both verbally and non-verbally required. LICENSURE, REGISTRATION, COMPLIANCE CHECKS AND/OR CERTIFICATION: Successfully complete &/or comply with all required background checks to include but not limited to: Ohio BCI&I fingerprint analysis, FBI (if applicable) fingerprint analysis, Nurse Aide Registry, HHS/OIG list of Excluded Individuals/Entities (LEIE) & Ohio Medicaid Provider Exclusion and Suspension List. Valid License to practice as a LPN in the State of Ohio. CPR certification required. RESPONSIBILITY FOR PATIENT CARE: Provide services based on physician orders. Communicate physician, patient and family needs with the appropriate caregivers. COMPUTER: Intermediate personal computer skills including electronic mail, routine database activity and word processing. LEADERSHIP RESPONSIBILITIES: Responsible for the direction and management of the STNA’s and/or Restorative Aides within the facility. RESPONSIBILITY FOR CONFIDENTIAL MATTERS Works regularly with some information such as medical records or reports, financial reports, information on computer screens or other electronic devices, and records that are restricted to specific persons.
