Conducts and individual patient assessment and on going assessments, develop pan of care, performs in home supervised visits every 90 days. Documents patient assessment findings, Maintains confidentiality in matters related to patient,and family, non-discriminatory manner that is sensitive to the patients and family. Reports patient condition to appropriate personnel during each sift. Responsible for management and supervision of clinical staff and services offered by the agency. Responsible for skill verification/demonstration of C N A/P C A task skills. Perform quarterly evaluation of client records.
