RN Case Manager-In Office (Elk Grove)

Registered Nurse (RN) Job Description Our Home Health Agency provides compassionate and high-quality care to our patients. We are seeking an RN Case Manager with clinical judgment experience to join our team! 90% of our patients are pediatric from birth to 21 years old. Patients are in a stable condition which allows them to be in the comfort of their home. Examples of our patients' DX are long-term diseases, and genetic disorders, at times they have trach, vents, and g-tubes. Depending on RN experience the hourly rate starts at $35.00+ (NEGOTIABLE) We are looking forward to working with you to help us take care of families in the areas: -Elk Grove -Sacramento Position Overview & Responsibilities: § Conduct initial assessments of patient's health conditions and create individualized care plans. § Regularly assess and reassess patients' needs and update care plans as necessary. § Educate patients and their families on proper care techniques and medication management. § Monitor patients' progress, document all care activities, and maintain accurate patient records. § Evaluate the safety and cleanliness of patients' homes and make recommendations for necessary modifications. § Ensure compliance with all applicable healthcare regulations and guidelines. § Participate in case conferences and team meetings to discuss patient care and coordinate services. Qualifications: § Excellent clinical assessment and critical thinking skills. § Strong communication skills. § Proficiency in electronic health record systems. § License RN: 1 Year (required) Required Credentials: § State license to practice in California as a Registered Nurse § CPR certification or BLS § Physical (within 1 year) § Negative TB (within 1 year) § Malpractice Policy (NSO) Work Location: In Office Job Type: Full-time Hourly Pay: $35.00+ /hr to (NEGOTIABLE) Expected hours: No more than 40 per week Medical specialties: Home Health Pediatrics Schedule: Day shift Monday to Friday Experience: RN: 1 years (Required) License/Certification: California Registered Nurse License (Required) Work Location: In personThis is a great opportunity to provide powerful training one on one with Patients and their families.
