We are seeking an experienced RN Case Manager to join our team and help make a difference in the lives of children and their families. About Us: At Alpine Home Health, 90% of our patients are pediatric, ranging from birth to 21 years old. These children are in stable condition, allowing them to remain in the comfort of their homes. Our patients often have long-term illnesses or genetic disorders and may require care for tracheostomies, ventilators, or g-tubes. Position Overview: As an RN Case Manager, you will play a pivotal role in ensuring our patients receive the highest level of care. Your responsibilities will include: Conducting initial assessments and developing personalized care plans. Regularly reassessing patients’ needs and updating care plans as required. Educating patients and their families on care techniques and medication management. Monitoring patient progress and maintaining accurate documentation. Evaluating home environments for safety and recommending necessary modifications. Ensuring compliance with healthcare regulations and guidelines. Participating in case conferences and coordinating with the healthcare team. Qualifications: We are looking for a skilled RN with: Strong clinical judgment and critical thinking abilities. Excellent communication skills. Proficiency in electronic health record systems. At least 2 years of RN experience (required). A valid RN license in California. CPR/BLS certification. Recent physical exam (within the last year). Negative TB test (within the last year). Malpractice insurance (NSO). Compensation: Competitive hourly rate starting at $40.00+ (based on experience, negotiable). Work Location: In-office role, with opportunities to serve families in areas such as Alameda, Berkeley, Fairfield, Fremont, Hayward, Marysville, Oakland, and San Leandro. Why Join Us?
