(RN) Registered Nurse - Essential Responsibilities
-Graduate of a nursing school.
-Must have a minimum 1 year of experience in Nursing
-Must be open to working a minimum 3 days out of the week (Sunday through Saturday)
-Experience in EMR software (preferred)
-Uses health assessment data to determine nursing diagnosis.
-Must complete an initial assessment of the patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illness(es).
-Assess patient needs, evaluate and modify care to meet goals of skilled nursing interventions.
-Collaborate with all case managers and DON to plan and evaluate team goals for each patient.
-Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
-Submit reports and documentation for each patient in accordance companies deadlines.
