Hospice RN Case Manager

We are seeking a person who believes their heart and talent is in the area of caring for patients who are terminally ill. As a RN Case Manager, you will provide nursing care and perform case management of the terminally ill hospice patient. You provide education to the family and patient regarding hospice services, home safety, end of life, and anticipatory grief. You will work as a valued member of the Hospice team in providing Hospice care within the interdisciplinary team that cares for the patient, Requirements: Must have a current RN license.
