Registered Nurse (RN) Job Description
Job description:
Pinnacle Home Care, LLC. is currently looking for Full Time or Per diem Registered Nurses (RN,) Case Manager to perform home health visits in the North West RI areas. This position is a Case Manager position where the RN oversees a team of LPN's and works within a multi-disciplinary team including PT and OT.
Full Time 32 hours- 40 hours / Per diem (Must be available to see at least 10 patients per week)
Mon- Friday (Rotation on weekends as needed with other RNs monthly) NO Nights
All Full Time employees are paid .43 a mile, patient to patient.
Responsibilities:
-Create and follow plan of care based off patients’ needs and doctors’ orders.
-Perform assigned duties, including administration of medication, wound care and treatments.
-Monitor patient’s conditions; reporting changes to Clinical or Client Services Manager
-Follow up with and properly document doctors' orders.
-Perform assessments as necessary.
-Case management and coordination.
-Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
Qualifications:
-Current license to practice as an RN in RI.
-Valid CPR license.
-Valid driver’s license and proof of insurance coverage.
-Ability and willingness to work in a team environment.
-Ability to travel between patient assignments within a 25-mile radius.
-Experience in Home Care a plus but not required.
-We follow all RI DOH mandates and will only accept candidates that are vaccinated -against Covid-19.

$40.00 per hour - $90 depending on Visit Type.
