Registered Nurse (RN) Job Description
**SIGN-ON BONUS OF $1500 AFTER COMPLETION OF 6 MONTHS OF SERVICE.** Responsibilities: 1. Functions as a role model by maintaining effective relationships with all team members. 2. Provides optimum client care in an efficient and cost effective manner. 3. Regularly assesses and prioritizes workload to ensure optimum productivity and quality of services. 4. Assures the safe delivery of therapeutic client care in accordance with policies and professional nursing standards. 5. Performs comprehensive, accurate physical assessments of clients, and records such data on agency forms properly. 6. Records thorough and accurate health histories and initial nursing assessments 7. Identifies client health problems accurately and comprehensively. 8. Communicates client’s conditions, responses and needs to medical practitioners and other professionals as needed to promote management of the client’s health needs 9. Participates in obtaining and tracking physician orders for services per agency policy. 10. Plans and performs interventions to ensure that needs are met in an appropriate and timely manner. 11. Sets realistic health goals for clients in concert with client and significant others. 12. Formulates and revises nursing care plans to consistently reflect the needs of the client in managing health problems. 13. Implements the nursing care plan with appropriate explanation of care and teaching to clients, staff and significant others as needed. 14. Teaches and demonstrates, insofar as possible, health care techniques or procedures to client/family. 15. Creates a positive environment conducive to team building and progressive nursing practice. 16. Participates in performance improvement activities in compliance with policy and professional standards. 17. Continuously assesses and evaluates the quality of services delivered in order to ensure the highest possible level of care and customer satisfaction. 18. Demonstrates assertive and creative problem solving ability by identifying problems affecting customer satisfaction, service quality and efficiency and makes recommendations for improvement. 19. Coordinates services with other disciplines, agencies, and services to meet client's needs in an efficient and cost effective manner . 20. Formulates paraprofessional care plans, if assigned. Orients and instructs the paraprofessional to the care plan, reporting requirements, and agency policy. 21. Supervises the paraprofessional. Performs evaluation, teaching, and records such activities as required by the agency, according to policy. 22. Updates and revises the nursing and paraprofessional care plan to reflect response to client change. 23. Participates in occupational health needs of the staff for WCAH including administering PPD/reading PPD and other treatments as required. 24. Informs the physician, the Field Nurse Supervisor and other personnel of changes in client condition. 25. Documents assessments, visits, communications, conferences, care plans and discharge planning and summaries by agency standards. 26. Completes admission and discharge paperwork in a timely fashion. 27. Completes all required documents for work time, time off, etc. in timely manner. 28. Participates in the nursing on-call schedule. 29. Consults, confers and/or reports to Field Nurse Supervisor regarding client issues and documents as necessary. 30. Identifies and plans for own learning needs. 31. Participates in the professional development of others. 32. Consistently performs in the best interest of clients and the agency and performs all related duties as defined in the NYS Nurse Practice Act from the Office of Professions. www.op.nysed.gov . Qualifications: 1. Licensed and currently registered to practice professional nursing with the New York State Education Department. 2.
