Position Qualifications:
Valid New York State RN License
2 years of experience in Homecare, and strong knowledge of LHCSA regulations
Ability to work independently
Must be familiar with current NYSDOH standards
Essential Job Functions / Responsibilities:
Open new cases/admissions
Responsible for review admissions and implementation of the care plan, ensuring it is within the scope of home health regulations
Oversee compliance with all state and federal regulations, laws, policies and procedures applicable to home health care services
Manage the development of educational programs through in-service and continuing education programs
Responsible for quarterly review and maintenance of accurate records in accordance with state and federal regulations
Develop and implement plan of care for clients in conjunction with client’s physician
Supervise, coach and support staff and HHAs responsible for the implementation of the designed care plan
