Registered Nurse (RN) Job Description
Experienced hospice Registered Nurse to take assignments for our patients residing in Whittier and Long Beach area. The hospice RN is responsible for providing care in the home to maximize the patient and family's quality of life. This candidate must have excellent phone, text and email communication with the team. Responsibilities and Duties The hospice RN’s goal is to work collaboratively with our Interdisciplinary Hospice Team with drive toward excellent outcomes for our patients care. The hospice RN has the following responsibilities: • Completes an initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. • Provides professional nursing care by utilizing all elements of nursing process. • Performs Supervisory Visit with Health Aides or LVNs to direct and review hospice care being provided accordingly to patient needs • Respects patients and families • Educate the hospice concept and the benefits of program and services to patients and families • Participate in IDG/IDT and updating of the Plan of Care Qualifications and Skills • Must possess a current and valid California RN license • 1 year minimum or more of experience within the last 2-3 years in Palliative Care, Home Health or Hospice is required • Must successfully pass required background checks • Must possess a valid driver’s license, provide proof of insurance, and have reliable transportation • Friendly, collaborative, outgoing personality • Must have excellent phone, text and email communication with the team Job Type: Part-time/Per Diem Rate: $85-100 per hour
