My dad was here for rehab and feeding needs. He has a feeding tube, they claimed they take good care of people with this. But anyway, I looked at the reviews before my dad was sent here. On Saturday, 11/15 my father had fallen. If his roommate wasn’t there he would have been on the floor for hrs. His roommate was hollering and screaming the nurse finally went to the room and said “why are you yelling?!” He was like look he fell. They were like oh like no urgency or anything. When he first got there he wasn’t hooked up to his feed for 12 hrs. DO NOT send your loved one here. They do not care. Not all nurses are rude and don’t care. I met 4 nurses that were really amazing and nice. But of course, they weren’t there when he had fallen. It was dirty and dingy. My poor dad’s sheets were never changed, they stole new clothes and blankets he had received.

this place is filthy and patients are left unkept. Even if this facility is functioning this way because a lack of staff it is inexcusable the conditions of this facility. If you even have a small amount of love for your family member please please please dont not under any circumstances send them here. As quick as my family got there they left. unacceptable. i have no idea how the state allows this. also the smell when you walk into the patient area hits you like a ton of bricks its vile the state needs to intervene this place is insanity

Worst rehab center I have ever been in Only 1 aide on the floor for all the patients because of this left laying in wet diapers, sheets,and clothing for maybe hours until someone can assist. Food is totally inedible which makes you vomit very frequently and you may want to set up delivery of food or have loved ones bring in meals. Be prepared to lose at least 5 to 6 pounds from the time you came in Staff has ups and downs there about 4 or 5 aides who care and do their best even being on floors alone. Nurses are only concerned for giving out prescriptions and not checking sounds

One of the worst in this state... The smell of this place can get u sick the staff the majority of them are just there for the check.... The way they speak to the people they care for is horrible... Don't know they pass health codes human codes seems like the state doesn't care what happens to the people there smfh. . WARNING THIS PLACE SHOULD BE SHUT DOWN GLAD MY FATHER IS NOT THERE ANY MORE!!!!!! DO NOT BRING YOUR LOVE ONES HERE..

Sapphire is dangerous and filthy, they make patients sit around in their own filth, don't clean the rooms, don't report/record medical episodes such as seizures, and more. I wouldn't put my worst enemy in this place.
