Caregiver Job Description
Education and Experience Required: State Certified Nursing Assistant Certification or Personal Care Assistant. Current CPR (American Heart Association). Prior experience and training in positions similar to that of the Direct Care Worker or with the disabled and youth population and their families faced with emotional and behavioral challenges. Minimum Physical Requirements: Must be able to go up and down stairs; bend and lift; and perform physical restraints. Position Summary: This position supports Lillie’s Healthcare LLC community-based Home Health Agency. The DCW is tasked with implementing the individual treatment plans on behalf of the clients of the home. The position is designed to maximize consistency of approach and to promote the greatest degree of developmentally appropriate independent functioning, so that, in most cases, each client may have the best quality of life. In addition to teaching self-help and basic living skills, a DCW helps clients achieve all of the competencies necessary to manage life events successfully. This will often include the development and use of coping strategies needed to manage emotional difficulties as well as the use of crisis management strategies. Day to day household management is incorporated into the expectations of this position, including but not limited to designing and participating in recreational activities, food preparation, and minor household maintenance, housekeeping, and assurance of client safety. Performance Standards DCW’s are expected to: 1. Provide the appropriate level of supervision for clients in their care and adhere to the individual service plan. 2. Demonstrate safe behavior and works to assure the safety of clients and peers. 3. Administer medication according to policies and procedures. 4. Providing physical support for clients with daily activities and personal hygiene, including bathing, dressing, getting out of bed, using toilet, walking, standing or exercising. 5. Obtaining a wide range of information from physicians, caregivers and nurses about patient condition, treatment plans and suggested activities. 6. Recording vital signs, including blood pressure, pulse, temperature and respiration rate as requested by staff. 7. Maintain, through communication and documentation, positive and collaborative relationships and continuity with other care providers. 8. Report to work on time and, if unable to report to work or if there is a need to arrive late or leave early, follow Lillie’s Healthcare call out procedures. 9. Consistently engage in regular supervision, implement supervisory recommendations and directives, seek supervision as needed, and be receptive to feedback. 10. Embrace change and actively participate in efforts aimed to improve the quality of services, operations or outputs. 11. Positively contribute to staff consultations, coordinating efforts with educational, treatment programmatic administrators and other staff to follow the ISP’s and help clients meet their goals. 12. Display patience, cooperation, professional demeanor and tact. 13. Maintain clear, concise, and current documentation. 14. Supervise clients assigned. 15. Participate in scheduled On-Call duties as needed. 16. Other responsibilities as deemed necessary by the supervisor. 17. Prepare meals daily.
