Caregiver Job Description
Caregiver
Responsibilities includes but not limited to:
• Participates in development of the individual service plan (ISP) for each assigned individual.
• Prepare and implement activities and plans meaningful community outings with the Individual’s support goals and objectives identified in the service plan.
• Plans community integration while adhering to the Individual Service Plan.
• Provides individual appropriate activities in one or more of the following areas: self-help, language development, perceptual motor development, simple academics, and grooming/hygiene.
• Assists with food preparation, service and cleanup, providing supervision and assistance to individuals as needed.
• Transports individuals to and from the Day Center and on community outings following
all agency transportation, safety, and vehicle operation policies.
• Assures that medications are available for self-administration by individuals as refills are needed.
• Maintains required medication records.
• Prepares documentation of individual activities that is complete, accurate, and legible and meets organization and accreditation requirements.
• Maintain files of all materials in prescribed format.
Requirements:
Must have 3+ years’ experience dealing with severe challenging behaviors
Must be at least 18 years’ old
Must be able to pass a NCIC Criminal Background
Valid Georgia Driver’s License
Clean Motor Vehicle Report
High school diploma or GED and combination of training and experience
One-year experience in social work or direct care services to special needs individuals in
Human services setting
Experience working one-on-one with special needs individuals
Or an Associate degree in a related field
