Certified Nursing Assistant (CNA) Job Description
JOB SUMMARY: In the capacity of on-site Staff, Live In staff will provide services to individuals with disabilities as determined by a plan of care in a (24) Twenty Four Hour Medicaid Waivered Facility. Manager will be under the direct administrative supervision of the Program Administrator for James L. Williams, LLC JOB DUTIES AND RESPONSIBILITIES: 1. Provide individuals in-home assistance with basic personal care such as toileting and grooming activities, bathing, hair care, assistance with clothing or exercise. 2. Provide individuals in-home assistance with medications/health care. 3. Provide in-home assistance with nutrition such as menu planning and meal preparation. 4. Staff to be responsible for all documentation related to the accomplishment of these duties. 5. Provide in-home assistance with other personal care activities as assigned. 6. Transport individuals to and from work; grocery or personal shopping; bill paying; doctor appointments; dentist appointments; social service or social security appointments; banking, etc. 7. Be a member of the Individual Service Plan (ISP) team by attending ISP meetings and instructing individuals on self-help skills as documented in the ISP; observes individual progress/behavior and documents data; guides individuals in accomplishing ISP objectives by demonstrating techniques and reinforcing good performance/discouraging inappropriate behavior. 8. Participates in program operation by documenting behavior, activities and progress on data sheets; completes various reports and forms; maintains order among individuals by promoting and reinforcing helpful and friendly behavior. When necessary, use Crisis Prevention Intervention (CPI) disciplinary procedures; order supplies; assist individuals in daily maintenance of living areas; identify maintenance/repair needs to Administrator and/or call for assistance; maintain a safe, clean, hazard free environment. 9. Have knowledge of independent living skills, "Becoming Independent." 10. Evaluate and determine individual's strengths and needs. 11. Attend staff meetings and other in-service training offered by James L. Williams, LLC and workshops outside of James L. Williams, LLC 678.435.4622
