Caregiver Job Description
THE COMPANY: Social Vocational Services (SVS) has been serving adults with intellectual and developmental disabilities throughout California since 1977. THE POSITION: We’re looking for full-time Community Inclusion Facilitator Duties include (but are not limited to): •Supervise and monitor clients during daily activities (including, but not limited to: paid work, social skills building, art, recreation, volunteering) •Develop & monitor individual client plans within the Program. •Assist with personal hygiene needs as needed. •Assess, evaluate, implement and chart client’s progress on goals and objectives •Serve on Clients' Person-Centered Planning Committee •Record data and maintain field books •Assist clients with personal and hygiene issues when needed •Provide constant supervision to clients MINIMUM JOB QUALIFICATIONS: •High school diploma, GED or equivalent •Good driving record and valid California driver’s license •Must obtain DOJ/FBI fingerprint clearance •Must pass post-job offer job-related physical exam including TB test •First Aid/CPR certification (SVS will provide) •Effective written and verbal communication WHY SHOULD YOU APPLY? •Health, dental and vision insurance •Paid holidays •Paid time off •5 day work week •You get to help our clients meet their individual goals and aspirations!
