Licensed Vocational Nurse (LVN) Job Description
Pay: $17.00 - $18.00 Job description: Job Summary: As a PAS Supervisor at Hoping Hearts Home Care, you play a critical role in fostering communication and collaboration between Owners/Officers, staff, and the community. This leadership position involves coordinating and overseeing caregiver management and administrative functions, ensuring daily operations are conducted seamlessly while upholding the agency's commitment to quality care. You will be instrumental in achieving strategic objectives aligned with the company's business and financial goals. Key Responsibilities: Demonstrate efficient teamwork with the staff. Understand and perform the admission process plan of care. Coordinate services and schedule client visits and supervise field staff as indicated. Provide in-service education per PAS Supervisor direction. Attend seminars, conferences, workshops, and self-studies to enhance leadership skills. Perform the initial home care visit and re-evaluate client needs and progress when needed. Notify the physician of a change in the client’s condition. Coordinate and monitor all client care and services and complete paperwork timely and efficiently. Maintain client records according to policy and procedure. Follow infection control policy in and out of the office. Provide quality care to clients in their homes according to policy and procedures. Participate in the on-call schedule and participate in and support quality improvement programs. Organize and set up monthly meetings with caregivers to discuss updates, address concerns, and foster a collaborative working environment. Distribute quarterly surveys to caregivers to gather feedback on their experiences and identify areas for improvement. Answer work calls promptly, addressing inquiries from caregivers, clients, and other relevant stakeholders. Assist in the timely filing of incident reports, maintaining accurate records, and ensuring compliance with company protocols. Manage and organize hospitalization and discharge records for clients, maintaining confidentiality and accuracy. Conduct regular audits of client files to ensure completeness, accuracy, and compliance with regulatory standards. Fill in during periods of staff shortage with clients, ensuring continuity of care and support for clients. Facilitate the training of new caregivers, providing guidance on company policies, procedures, and expectations. Coordinate special department activities, including conferences, workshops, and other functions. Monitor the record system and statistical reporting system for proper documentation, planning, and evaluation. Develop policies focused on client care in collaboration with staff and Owners/Officers. Oversee care delivery and client outcomes to ensure they meet clients' needs. Monitor and ensure client rights are exercised, compliance with laws and standards, and adherence to policies and procedures. Ensure personnel availability and qualifications meet the needs of clients' plans of care. Assure the accuracy of public information material and activities. Implement performance improvement priorities. Qualifications and Requirements: ****MUST be CNA or LVN certified**** Possess a high school diploma or GED with at least one year of experience or training in caring for individuals with functional disabilities. Have completed two years of full-time study at an accredited college or university in a health-related field. Excellent communication and interpersonal skills. Strong organizational and multitasking abilities. Proficient in handling administrative tasks and maintaining detailed records. Complete a minimum of twelve (12) clock hours per year of continuing education in relevant subjects.
