Home Health Aides

Job Summary We are seeking for Home Health Aide (HHA) candidates who are compassionate, caring, reliable, self-motivated, detail-oriented, flexible to work in different settings, excellent communication skills with experience in Home Health. The HHA will provide assistance to the RN in executing and implementing the developed plan of care of the patients. Responsibilities • Perform individualized treatment tasks established by RN based on patient evaluations and established goals on plan of care. • Perform activity of daily living not limited to: • Bathing • Grooming • Meal preparation • Light Housekeeping • Companionship • Educate patients and their families about treatment plans, exercises, and home care strategies established by RN or other clinicians based on the plan of care. • Document and complete patient visit notes and necessary documentation in a timely manner to assure compliance with company policy and Home Health standards. • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Skills & Qualifications • Current HHA certifications • Current CPR certification. • Experience in treating patients in home settings. • Ability to work independently and remotely. • Excellent communication skills for educating patients and collaborating with healthcare teams. All positions require a Level 2 background screening. Demonstrate all professional trainings and certifications.
