LPN's Needed

The Licensed Practical Nurse provides skilled nursing care to assigned patients. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with nursing standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned: Provides direct and individualized nursing care to assigned patients based on nursing standards and reports activity as required. • Ensures quality and safe delivery of home health services to patients/clients. • Implements plan of care formulated by physician and Registered Nurse. • Provides accurate and timely documentation consistent with plan of care. • Provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care. • Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards. • Assists patient in learning appropriate self-care activities. • Uses equipment and supplies effectively and efficiently.
