Licensed Vocational Nurse (LVN) Job Description
LVN with previous home health or hospice experience preferred but willing to train. Candidates must possess good assessment skills, basic documentation with computer skills, the ability to travel to the patient's home, and good communication skills. Must have a good understanding of death and dying and end of life care philosophy; post-acute delivery care models for effective, proactive, and efficient management of patients in their own homes. Requires the ability to use a computer and navigate EHR.
Must have at least 1 year experience in any health care settings for the past 24 months preferably in home health, hospice, acute care or nursing homes; dependable transportation; able to demonstrate proficiency in direct patient care skill especially in end of life care management.
