Home Health Aide (HHA) Job Description
We are seeking Certified Home Health Aides to become a part of our team! You will provide a combination of personal hygiene duties and light housekeeping for patients in a variety of settings, including private homes, nursing homes, assisted living facilities, and other care facilities in accordance with the Plan of Care prepared by an RN. Responsibilities: 1. Provides routine individualized care to patients. 2. Provides personal care such as bathing, dressing and grooming, hair care and oral care according to the patient's Plan of Care. 3. Supports patients by providing light housekeeping, preparing and serving meals and snacks. 4. Changes linens to ensure patients are comfortable and clean. 5. Spends time talking and listening during bedside visits. 6. Monitors and records changes in patient health status and notifies the RN Case Manager of changing or unusual conditions. 7. Helps family members care for the patient by teaching appropriate ways to lift, turn, and re-position the patient. 8.Maintains a safe, secure, and healthy patient environment by following asepsis standards and procedures, maintaining security precautions, and following prescribed dietary requirements and nutrition standards. 9. Most importantly, makes a difference in patients’ lives towards the end-of-life. 10. Protects the integrity of the hospice by adhering to professional standards, home care policies and procedures, federal, state, and local requirements, and the hospice protocols. Qualifications and Skills Requirements 1. Must have a valid CHHA certificate in the State of California. 2. Positive and compassionate attitude 3. Ability to build rapport with patients 4. Ability to handle physical workload. 5. Written and verbal communication skills 6. Timely and accurate documentation/record-keeping skills 7. Must be able to travel to jobs per each assignment
