Licensed Vocational Nurse (LVN) Job Description
Job Description We are urgently hiring hospice Licensed Vocational Nurses (LVN) to join our team on part time/per diem and full-time basis. We are looking for LVNs who are dedicated to patient care, compassionate, exemplify positive attitude, thrive in a fast-paced environment and can work well with all kinds of people . We provide our staff with a strong foundation for hospice care through training, education, and mentoring in a supportive environment to ensure their success. Hospice LVN Field Nurse is primarily responsible for providing direct patient care under the supervision of a Registered Nurse (RN). Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. This position enables patient to receive the best quality of care through appropriate nursing interventions in accordance with plan or care. Responsibilities and Duties: • Demonstrates competency in nursing and assessment skills • Accurately identifies and communicates any changes of condition that warrant adjustments in the plan of care • Executes nursing intervention as indicated in the plan of care approved by the RN/Case Manager • Exemplifies excellent patient care and professionalism • Provides adequate education on appropriate usage of medications, DME and supplies • Coordinates hospice services as needed, appropriately reporting the identified needs to the interdisciplinary group • Communicates with family, caregivers, hospice team members for any changes in patient condition and commences plan of care as needed and approved by the RN • Performs other duties that may be assigned by the Registered Nurse/Case Managers. • Provides accurate and timely documentation of patient services to reflect the plan of • Performs nursing job in compliance with federal and state applicable laws and standards and the hospice policies and procedures • Maintains confidentiality in accordance to HIPAA compliance Job and Skills Requirements: • Current California LVN License (required) • Current CA Driver's License with a clean driving record (required) • Valid CPR certification (required) • Proof of auto insurance (required) • Minimum 1 year experience in home health and/or hospice (preferred, not required) • Reliable Transportation • Must have excellent communication skills, verbal and written • Must have the ability to work well within a team Bilingual: Bilingual is a plus! Schedule: • Monday to Friday • Weekends
