Hospice RN Case Manager - Palmdale, Los Angeles, Riverside.

We are urgently hiring Hospice RN on per diem basis to join our team! We are looking for RNs who are dedicated to patient care, have the ability to thrive in a fast-paced environment, and can work well with others. We provide our staff with a strong foundation for what we do through training and education, and give them the support necessary to ensure their success. Job Responsibilities: 1. Performs and completes an initial, comprehensive, and ongoing assessments of patient and family to determine patient needs. 2. Creates, organizes, and manages individualized patient care plan in coordination with other members of the hospice team to meet the needs of the patient. 3. Provides direct hospice and palliative patient care and clinical interventions in accordance with the Plan of Care and to meet the needs of the patient. 4. Works closely with the patient and the Interdisciplinary group to meet the goals of the patient. 5. Supervises Health Aide and LVNs in providing care to patients. 6. Provides leadership to assigned hospice team/s and advocates for patient. 7. Monitors and reports changes in patient symptoms or behavior and updates the POC achieve patient goals and outcomes. 8. Educates patients and family about patient's health maintenance and disease prevention. 9. Attends IDG meeting to discuss the condition of the patient. Requirements/Skills * Active RN License in CA (preferably with a BSN degree) * Minimum of one year experience in hospice and palliative in a hospice agency * Strong clinical skills * Willingness to do admissions, continuous care, supervisory visits, PRN and routine visits * A strong foundation of acute care experience. * Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient locations. * Ideal candidates must have excellent communication, interpersonal and decision-making skills; commitment to excellent hospice patient care; and familiarity with federal, state hospice regulations and CDC guidelines.
