Licensed Practical Nurse (LPN) Job Description
We are seeking a Resident Care Coordinator to join our team! You will assist in the daily care of residents at our HIV/AIDS Adult Family Home in lovely Bellingham, WA. This position is 20 hours per week to start, and can increase to 40 hours per week in 3-4 months. Responsibilities: • Supervise and train caregivers in an adult family home environment • Assist residents with daily living activities and provide life skills, behavioral, and medical case management • Communicate, coordinate, and schedule resident appointments and medications • Collaborate with residents and families for best care opportunities • Oversee maintenance of a healthy, safe, and comfortable living environment • Work closely with oversight agencies (such as DSHS) when needed • Work as part of a 3-person management team, sharing coverage and responsibilities as needed • Oversee and monitor staff performing menu planning and preparation • Provide occasional care coverage in the event of staff vacancies • Read, understand and adhere to the Washington State Administrative Code (WAC) for Adult Family Homes • Act as Entity Representative after paid training is completed • As part of the management team, review applicants’ eligibility for admission and residents’ need or appropriateness for discharge, in accordance with WAC and RCW requirements Qualifications: • Have completed at least 1,000 hours of successful direct care experience to vulnerable adults in licensed or contracted settings in the previous 60 months OR hold one of the following licenses: physicians, physician assistants, registered nurses, or licensed practical nurses • Have no disqualifying criminal convictions or pending criminal charges under chapter 388-113 WAC • Strong written and verbal communication skills
