Licensed Practical Nurse (LPN) Job Description
Job Title: Charge Nurses (RN-LPN) Department: Nursing Supervisor: Director of Nursing Supervision Given to: CNA’s Working Hours: Full time forty (40) hour per week. These may be week days and/or weekends on any shift. Minimum Qualification: Must be at least twenty (20) years of age, a graduate of an accredited School of Nursing, and be currently licensed in the state the facility is located, organizations and leadership ability, prefer one (1) year experience in Long Term Care nursing, genuine interest and concern for working with the elderly. Job Summary: Responsible for the planning, organizations and implementation of daily nursing assignments to ensure the delivery of care to the residents within their area of supervision. This includes the assignment of breaks and meal times. Job Duties: 1. Responsible for the assignment of job duties for the CNA’s in accordance with their level of training and the residents needs, including breaks and meal times. With follow up supervision to ensure compliance. 2. Assist in the development and establishment of the residents MDS and Care Plan. 3. Responsible for nursing documentation which includes the monthly summaries (incorporating the resident Care Plan). Acute reactions to medication, treatments accident and/or incidents. 4. Knows and understand the principles of good body alignment, prevention of decubiti and nutritional awareness. 5. Investigates accidents, injuries and incidents involving both residents and staff. Prepares and completes necessary report for same. 6. Communicates with the Director of Nursing and/or Administrator regarding the current status of residents on an as needed basis. 7. Assist family and visitors with questions regarding their family member. Directs the family and/or visitor to the Director of Nursing as indicated for more detailed information. 8. Disciplines CNA’s as needed in accordance with personnel policies. 9. Responsible for knowing and ensuring all residents’ rights are maintained. 10. Interacts with the Director of Nursing on required evaluations of those CNA’s who are on their assigned shift/wing. 11. All other duties that are deemed necessary by the Director of Nursing and/or Administrator. 12. Responsible for making rounds on all residents to evaluate, receive comments and reviews care being given to ensure residents are maintained at their highest level of functions. 13. Responsible for verifying the medication administration records (MAR’s) and treatment records (TAR’s) are completed daily. Responsible for: 1. Treating all residents, employees, visitors with respect and dignity. 2. Knowing and protecting all residents’ rights. 3. Knowing and enforcing the facilities policies and procedures. 4. Participation in fire/disaster drills, in-services scheduled staff meeting and any workshop/education programs deemed necessary
