Responsible for knowing and demonstrating St. Otto’s Values while performing all job functions. Operationalizes values with residents, visitors, and other employees. 2. Follows all safety procedures, policies and manufactures instructions when applicable. Responsible for prevention (or proactive) of unsafe work situations. Participates in safety program. Is familiar with general and departmental safety requirements. Attends training and in-service opportunities. Attends Falls Committee Meetings as able. 3. Provide nursing care utilizing the components of the nursing process and evaluating residents care through the concept of a modified primary nursing care delivery system. Responsible for managing, along with LPN or RN preceptor, resident care on a day-to-day basis. Participates, along with LPN or RN preceptor, in the development of a comprehensive Resident Care Plan involving the Resident, Family, Nursing Assistant and interdisciplinary team. Responsible for the periodic review and proposing interventions to update the care plan to ensure the resident attains and maintains the highest practicable physical, mental & psychological well-being. 4. Identifies and actively supports the role of resident advocate by communication with the interdisciplinary team. Documents in resident’s chart communication and plan of care with interdisciplinary team. 5. Assist RN, LPN, and HUC preparing for physician rounding 6. Assist RN and LPNs with Admissions and Discharges 7. Assisting RN Supervisor with verification of orders 8. Involvement in Quality Improvement Initiatives (Reduction of pressure ulcers, monitoring of psychotropic medications, fall prevention, restorative nursing) 9. Assisting RN’s and LPN’s with updating physicians in change in conditions 10. Contact on call physician 11. Supervision by RN or LPN with assessments of lung sounds and edema 12. Assisting RN with Wound Rounds 13. Assisting RN and LPNs with dressing changes 14. May be delegated simple dressing changes with training 15. 1:1 with residents that have increased behaviors 16. Completing tasks and treatments 17. Assessing a fall; documentation of event and follow up interventions. Involvement in Falls Interdisciplinary Team Huddles and Follow Up 18. Calling families if need be 19. Responding to Alverna calls 20. Assist checking in medications from pharmacy 21. Training in and administration of insulin 22. Completion of training on catheter insertion and changing of catheters training per competencies 23. Training on edema checks 24. Training on dressing changes 25. Nursing clinical documentation in progress notes to be signed off by RN or LPN preceptor 26. Answering call lights and assisting CNAs with resident care 27. Functions as a positive role model in professional relationships and interactions by utilizing and sharing leadership skills appropriate to the role of an LPN intern within the facility. 28. Participates in the control of cost-effective services that include proper use of equipment, appropriate use of supplies, and effective, efficient organization of resident care duties. 29. Demonstrates interest in and commitment to assuring resident and family satisfaction. 30. Identifies needs and concerns involving safety/risk management and quality improvement issues. Attends Quality Assurance Performance Improvement Committee meetings as able. 31. Assumes responsibility for enhancing personal and professional development. Attends monthly nurses’ meetings. Maintains compliance with additional required trainings. 32. Responsible for maintaining appropriate attendance, health, and hygiene according to departmental and position standards. Is punctual in arriving to work, meetings, and following other schedules. Always maintains confidentiality.
