Licensed Practical Nurse (LPN) Job Description
Homeward Bound On-Site Supervisors are responsible for safely and professionally providing exceptional programming and life enrichment services to the people we serve. On-Site Supervisors are also responsible for leading others through training, coaching and mentoring Direct Care Staff. Essential Functions This job will be expected to complete the following functions. This is not an all-inclusive list. This job may be assigned other functions or tasks as business or client needs dictate. This is meant to be a general description of the most essential functions only. Life Enrichment Program Implementation Seek to understand each individual, their goals, dreams and interests, and support them in living their best life! Read, comprehend and implement individualized program plans, health care plans, and all other individual care plans as written. Participate in and implement Person Centered Planning activities. Participate in providing high quality personal care and program services to individuals served. Ensure the delivery of active treatment by training, directing, and supporting individuals served in the completion of their daily living skills, including: hygiene, grooming, dressing, eating, communicating, administering medications, and activities both within the home and in the community. Ensure the protection and promotion of basic civil, human and legal rights of all individuals served. Ensure the accurate, comprehensive and faithful recording of data on the individual’s GPN’s, MAR’s, program documentation, incident and other reports or records about service delivery, approaches, goal performance, activities inside the home and outside in the community, and any other events or findings required to be documented. Promote Choice and Personal Interests Actively support individuals in making their own choices, directing their own care as much as possible, and pursuing their interests. Offer or suggest new things on occasion. Ensure individuals go on fun and interesting outings frequently. Suggest options for outings that may be of interest to individuals. Assist them in selecting and planning outings. Encourage and facilitate community inclusion for each individual. Community inclusion can take the form of friendships, clubs, employment, teams, groups, etc. (To be “included” suggests when someone is gone they will be missed by others.) Transport and accompany individuals to all appointments and outings. Support individuals with observing their personal religious beliefs and practices such as accompanying them to church and assisting them in celebrating religious holidays. Daily Living Ensure the understanding, expression, protection, and promotion of the preferences and choices of individuals in the daily provision of support, assistance and health care. Actively engage in conversation and activities with individuals at all times. Encourage individuals to participate to the fullest extent possible in completing activities of daily living including meal preparation, bathing, toileting, personal hygiene, dressing, eating, communicating, Passive Range of Motion, transporting, mobility and involvement in activities at home and in the community. Complete personal hygiene activities as required, including but not limited to: changing briefs (freshening), bathing, toileting, toothbrushing, nail trimming, haircuts, etc. Prepare and serve meals according to each individual’s health and dietary needs. Assist with ensuring the house is appropriately stocked with required food to prepare meals according to the house menu. Complete and record accurate data on individual programs, goals and activities. Ensure the handling and safeguarding of all funds belonging to individuals or the organization. Complete appropriate documentation and follow through on employee incidents, individual incidents, HBI incidents and Vulnerable Adult/Child Protection issues as required by company
