Patient and Staff Care Coordinator is responsible for assisting the DON and in QA Manager with:
1. Clinical Management; intake, communication with patients, staff, contracted providers, referral sources, and physician offices (40% of the time)
2. Records Management; assist with clinical and staff records and training compliance.
3. Patient Visit; functions as a visiting staff reliever.
This is a full-time position, Monday to Friday from 8:00 am to 5:00 pm.
Requirements:
Current Active CA LVN License
Current CA DL
Clean State and Fed Background
Second Language: Spanish
