Bring your skills and talents to a role where you’ll have the opportunity to create Value and Make a Difference in People’s lives Job Description : This position will provide personal care service to the terminal ill patient and perform related tasks in accordance with the plan of care as assigned delegated by the Director of Patient Care Services . You will be performing in-home admissions, visits and provide care and support through assessing, planning, implementing and monitoring patients during their end of life journey. We seek an experienced Registered Nurse who is knowledgeable in Hospice and home health. Team-oriented, compassionate, patience, and providing emotional support are vital in this role. Service area: Ventura County Qualifications: Licensed California Registered Nurse in good standing At least 2 years of supervisory / case management experience in Hospice and Home Health with active patient with in the past 3 years Computer proficiency and working knowledge of EMR Current CPR certificate Current Fingerprint Card Valid Driver’s License and proof of insurance; with reliable transportation to travel to patient homes or other locations within service delivery area Work weekends and holidays Naturally compassionate, understanding and can relate favorably to patients and families and to work cooperatively with other employees Exceptional interpersonal skills and has passion for Hospice Knowledge of the death /dying and bereavement process Bilingual English/Spanish is a plus Angel of God Hospice and Palliative Care Expectations: Compliant with all Hospice policies and procedures. Under the physician’s order, admits patients eligible for hospice services . Assess and evaluates patients needs/problems,Identifies mutually agreed upon goals with patient while working closely with the interdisciplinary Group. Provide Intermittent skilled nursing services including assessment ,evaluation, procedures, teaching and training activities as outlined in the patient IDG PLAN OF CARE. Maintain absolute confidentiality of all information pertaining to patient, families and staff and adhere to all HIPAA rules and regulations. Demonstrate ongoing responsibility for self-education. Participate and promote employee job safety analysis to eliminate hazards and prevent accidents. Participate in the Quality Assessment Performance Improvement Program (QAPI), as requested. Participate in an on call system and is responsible for providing on call coverage when unavailable for assigned duties. Why work with us?
