Registered Nurse (RN) Job Description
Nurse Case Manager is responsible for managing each episode of care including: admission, planning, assigning, directing and monitoring progress of home health care services, and discharging patient from care. The RN Case Manager is experienced in nursing, with emphasis on community health education. The professional nurse case manager builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Compensation
$65.00 - $75.00 per hour
