Licensed Vocational Nurse (LVN) Job Description
Qualifications: - Current and valid license for L.V.N. and/or L.P.T. for the State of California - Proper education and experience Responsibilities for Staff Nurse: - Administratively reports to the Charge nurse - May function at Charge nurse with approval of Director of Nurses - Assist physician with treatments and procedures - Instructs residents concerning ongoing care with HIPAA guidelines - May initiate appropriate emergency measures, within scope of practice - Participates in or leads group therapy, individual therapy as needed - Participates in unit conferences, committee meetings, staff development, and in-service programs - Maintains resident records and charts and may perform other appropriate record keeping activities o Develop and implement plan of care - May prepare medication according to policy and administer intramuscular, intradermal, subcutaneous, and oral medications - Ensure infection control measures are in place and adhered to - Insure resident and family privacy, dignity, comfort, and safety - Report any changes of resident condition to charge nurse - Perform other nursing duties as assigned by Charge nurse - Supervise CNA’s on floor as directed by Charge nurse - Will promote and assure residents safety or maintain higher practicable physical, mental, and psychosocial well-being of each resident. Responsibilities for Medication Nurse: - Demonstrate knowledge of medication procedures: purpose, action, dosage, and side effects - Dispense medication from location of medication care in a safe area (i.e.: utility room) based on procedures for administration or medications at this facility - Dispense the right medication to the right patient with the right dose at the right time, and right documentation - Charts all medications on the patient’s medication sheet from physician’s orders. Check all medication cards against medication kardex. Night medication nurses will check the medication kardex and medication administration record against the physician’s order sheet in the patient’s chart. - Question any medication order that is unclear and clarify - Document on reverse side of medication sheet, chart PRN medication given, time, reason, and route given. Chart effect one hour after medication is given. - Keep the narcotics drawer double locked at all times - Keep working area clean - Report to charge nurse on unit observed reaction - Transcribe doctor’s orders; order medication from pharmacy - Report any medication dispensing error to the nursing supervisor/doctor - Report any error in supply or patient dose to the charge nurse and pharmacist - Pour medication at prescribed times only - Count narcotics each shift with the oncoming shift - Report any discrepancies in the narcotic count to the charge nurse - Updates, resolves, implement care plans, for medical and behavioral problems - Will promote and assure resident’s safety to attain or maintain highest practicable physical, mental, and psychosocial well-being of each resident.
