Licensed Vocational Nurse (LVN) Job Description As a home health nurse, your average day will look a bit different than a traditional nurse working on the hospital floor. Preferably with Home Health experience, excellent skills in patient care, compassionate, efficient time management techniques, maintains positive and respectful demeanor, able to maintain strict confidentiality, skilled in assessing and reporting in an accurate, concise and professional manner; educates patients and significant others; open to training and education; good communication skills; establishes and maintains positive interpersonal relations with internal and external clients; works well with the team. • Workplace Independence • Competitive Pay • Lower Stress Environment • Develop Meaningful Patient-Nurse Relationship ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care • Scheduling and coordinating care. • Performing field visits. • Reviewing documentation to ensure regulatory compliance. • Assessing the needs of patients & families, providing care per physician order, addressing symptoms, and ensuring comfort. • Developing and implementing a care plan to provide restoration of functional capacity, control of symptoms, and relief of suffering while respecting the patient’s goals and family values. • Anticipating disease progression and implications for symptom management. • Collaborating and communicating with the Interdisciplinary Team including physician when appropriate. • The ideal candidate enjoys working with a diverse team of health care professionals who are committed to providing excellent care, is comfortable with changing demands and priorities, is flexible, has excellent communication skills, and is a good problem-solver. Requirements • Must be eligible to work in the U.S. • ACTIVE LVN License Degree or work related experience. • BLS and Liability insurance is a requirement. • Driver License, auto insurance and reliable transportation is a requirement. • English speaking (Required). • Armenian speaking (Preferred). • Spanish speaking (Preferred). • Pay is per Visit and DOE.
