Registered Nurse (RN) Job Description
As a home health nurse, your average day will look a bit different than a traditional nurse working on the hospital floor. Preferably with Home Health experience, excellent skills in patient care, compassionate, efficient time management techniques, maintains positive and respectful demeanor, able to maintain strict confidentiality, skilled in assessing and reporting in an accurate, concise and professional manner; educates patients and significant others; open to training and education; good communication skills; establishes and maintains positive interpersonal relations with internal and external clients; works well with the team. • Workplace Independence • Competitive Pay • Lower Stress Environment • Develop Meaningful Patient-Nurse Relationship ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care • Completes an initial assessment of patient and family to determine home care needs. • Provides a complete physical assessment and history • Regularly re-evaluates patient nursing needs. • Initiates the plan of care and makes necessary revisions as needed • Uses health assessment data to determine nursing diagnosis. • Initiates appropriate preventive and rehabilitative nursing procedures. • Administers medications and treatments as prescribed by the physician. • Provides direct patient care as defined in the State Nurse Practice Act. • Counsels the patient and family in meeting nursing and related needs. • Provides health care instructions to the patient • Identifies discharge planning Requirements • Must be eligible to work in the U.S. • ACTIVE RN License Degree or work related experience. • BLS and Liability insurance is a requirement. • Driver License, auto insurance and reliable transportation is a requirement. • English speaking (Required). • Armenian speaking (Preferred). • Spanish speaking (Preferred). • Pay is per Visit and DOE.
