Role: Hospice Licensed Vocational Nurse (LVN) Job Location: Los Angeles, Orange County, Riverside, Kern County, Fresno County. Position: Freelance Salary Range: Depends on Experience SUMMARY We are looking for a Hospice Licensed Vocational Nurse (LVN) to join our team at Vela Hospice Care, Inc. In this position, you will focus on providing direct patient care under the supervision of a registered nurse. Responsibilities include visiting patients at various local residences and/or facilities, following the plan of care, providing treatments, timely documentation and reporting, and working collaboratively with the members of the team to help meet positive patient care outcomes. Responsibilities: Provides direct patient care as defined in State Nurse Practice / Vocational Act. Implements current nursing practice following patient's admission and initial assessment by Hospice RN Case Manager. Assists the RN Case Manager and /or Physician in carrying out the plan of care and performing specialized duties related to end of life care and coordination of palliative care services through scheduling, plan of cares and service coordination. Uses equipment and supplies effectively and efficiently. Provides direct patient care and nursing services, under the supervision of the Hospice RN Case Manager, to patient and family based upon the individualized written plan of care. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor. Provides accurate and timely documentation of patient services to reflect the individualized plan of care and outcomes of interventions and assessment of the patient/family response to care. Educates patient, family and caregivers in the support of the patient including pain and symptom management. Administers/prepares equipment and supplies effectively and efficiently for treatments requiring adherence to sterile or aseptic techniques. Provides appropriate pain / symptom management. Evaluates patient's response to treatments/ medications. Qualifications: Graduate of accredited practical/vocational nursing program, Associate degree preferred Valid LVN License in the state Proven experience as hospice care Understanding of the hospice philosophy, pain management and the needs of the terminally ill. Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order. Demonstrate excellent interpersonal and communication skills Solid organizational skills.
