Provides direct patient care. 2. Computer competency demonstrated through timeliness and accuracy of documentation. 3. Documents and reports observations, clinical findings, problems, skilled interventions, goals and discharge plans in accordance with Nursing & Rehab At Home standards. 4. Provides and documents skilled nursing care on all visits (includes skilled observation of the patient’s condition, skilled care and procedures and teaching of the patient/family. 5. Validates patient/family learning. 6. Documents in accordance with Nursing & Rehab At Home policy. 7. Executes therapeutic and/or supportive interventions. 8. Evaluates the patient/family response to and effectiveness of interventions. 9. Evaluates the patient/family progress toward established outcomes. 10. Communicates with the case manager, physician and other members of the home care team informing them of significant changes in the patient’s condition and needs. Documents these communications. 11. Follows policies related to MD orders and changes to the Plan of Care Requirements 1. Current Licensed Vocational Nurse licensure from the State of California. 2. An ability and willingness to demonstrate and maintain clinical competency as required in-home health care. 3. Must have a minimum of one year of nursing experience. 4. CPR certification 5. Excellent verbal and written skills 6. An ability and willingness to work with patients of various ages and needs. 7. A positive attitude towards health care team members and diverse patient populations.
