Licensed Vocational Nurse (LVN) Job Description
Mission Hospice & Home Care serves patients and families in San Mateo County and northern Santa Clara County with quality, compassionate end-of-life care and education. Founded in 1979, our local, independent nonprofit has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement. About the position: The LVN Triage Nurse is responsible for triaging patient, family, physician, and other calls related to the delivery of Mission Hospice & Home Care services, and processing and coordinating inquiries for admission. Responsibilities: • Triage hospice patient-related calls and provide appropriate patient teaching, assistance, and instruction on symptom management • Collaborate and report to hospice field clinical staff on patient needs • Provide information regarding Mission Hospice & Home Care services to referral sources including physicians, case managers, patients, and family • Screen and process inquiries for admission to Mission Hospice & Home Care programs to include confirming attending physician, entering verbal orders, assessing and coordinating DME needs, coordinating admission visit, and ordering symptom relief kits from the pharmacy • Serve as a member of the hospice clinical team to address problems associated with the delivery of patient care • Assign home health aides (HHAs) and manage HHA visit schedule as needed • Track death certificate worksheets and attestations and ensure that mortuaries and the health departments receive necessary and timely information related to patient deaths • Audit patient charts for completeness and accuracy of RN documentation • Remain flexible as to the duties of the position • Perform specific assignments as directed by the Hospice Program Director • Other duties as assigned Qualifications: • Current CA LVN license • Graduate of an accredited LVN nursing program • Current CPR certification • One year nursing experience, hospice experience preferred • Computer proficiency in Microsoft Office and willingness to learn new software as needed • Team player with excellent organizational, communication, and problem-solving skills • Demonstrated ability to interact graciously and relate effectively with patients, families, physicians, case managers, facility administrators, and community agencies • Understands hospice philosophy & comfortable providing specialized care to the terminally ill Please Note: The salary range provided here is only a reference guide but final salary rate depends on work experience.
