Registered Nurse (RN) Job Description

Employee benefits include: • Matching 401k • Medical, Dental, and Vision plans • Pet Insurance • Basic and Voluntary Life Insurance • Long Term Disability Insurance • 3 weeks of Paid Time Off (PTO) • HSA and FSA accounts • Employee Assistance programs • $5k sign-on bonus (with 3+ years RN experience) About the position: Manage the patient’s plan of care from after the initial admission visit until the patient’s death or discharge, coordinate plan of care communication with interdisciplinary team as necessary on patient or family assessments, report patient status along with admissions and re-certifications at biweekly IDT meetings, collaborate with physicians as necessary, and provide patient and family education regarding symptom management and other clinical needs. Responsibilities: • Assumes care management for patient/families involved with Mission Hospice in conjunction with physicians, the Interdisciplinary Team and other professional agencies • Assesses the patient /family in the home setting and implements the Interdisciplinary Group Plan of Care • Helps develop, evaluate and revise patient Plans of Care in conjunction with the Hospice Medical director and other members of the Interdisciplinary Group and the patient’s attending physician • Participates in Interdisciplinary Care Conferences • Drives to patient homes for regularly scheduled visits to the patient/family for provision of care, and for ongoing assessment of physical, psychological, social, and spiritual factors that may influence patient/family well-being; communicates new information to the Interdisciplinary Group, plans for bereavement follow-up with families • Attends in-service training workshops and conferences as needed to stay current with changing trends in nursing and hospice services • Keeps up to date in EMR documentation Qualifications: • Current CA RN license • Graduate of accredited RN nursing program • Current CPR certification • One-year RN experience or Mission Hospice Internship, with clinical experience in the care of patients with advanced illness, pain and symptom management • Valid California Driver’s License and an automobile that is insured in accordance with state requirements • Computer proficiency in Microsoft Office and willingness to learn new software as needed • Team player with excellent organizational, communication, and problem-solving skills • Knowledgeable of Medicare regulations • Ability to organize and prioritize workload independently • Understands hospice philosophy & comfortable providing specialized care to the terminally ill • Preferably fluent in at least one: Mandarin, Cantonese, Vietnamese language Physical abilities/requirements: • Ability to safely lift/transfer 50 lbs with or without assisted devices (patients, supplies, etc) • Ability to push, pivot, pull, and reposition 50 lbs with or without assisted devices • Ability to bend, squat, stoop, crouch, kneel, reach, climb, and rotate • Manual dexterity (grip/grasp/pinch) • Ability to stand for prolonged periods of time • Ability to walk for prolonged periods of time • Ability to drive for prolonged periods of time • Visual and hearing acuity with or without assisted devices • Withstand different work environments o Internal - office noise and interruptions, open workspaces o External - a number of elements including but not limited to various community & living environments, weather, & travel/traffic conditions Please Note: The salary range provided here is only a reference guide, but final salary rate depends on work experience.
